Confidentiality and Record Keeping

  • Basic personal information will be collected for contact and identification reasons. Notes will be taken during your appointments to support the delivery of a therapy service to you and because of the legal requirement to do so. 
     
  • All appointments or contact made are logged and entries are made into a confidential and secure system, and your therapy and personal information are kept securely.
     
  • Your information will be kept confidential. There may be occasions where the confidentiality agreement would not apply, to keep you or others safe or share information, such as criminal activity. This is because I have a legal duty to share certain types of information and to maintain yours and others safety as far as reasonably possible. In most situations, we would discuss this in the session and agree on an appropriate plan together. 
     
  • If you have not provided consent for me to liaise with your GP, then this may be overridden in the above circumstance
     
  • I ask that you are respectful of our confidentiality too. If you wish to record sessions, you must discuss this with me so that I can provide you with written consent. By signing this agreement, you agree not to covertly record the session or to allow someone to join the session without permission from your therapist.
     
  • It is important that I hold the most up to date information for you. Should your details change, please notify us as soon as possible.
     
  • You have rights relating to the information I hold to verify the accuracy or to ask for them to be supplemented, deleted, updated, or corrected. You have the right to request a copy of the information that I hold about you. If you would like a copy of some or all your personal information, please email or write to me via the contact details stated in this agreement.
     
  • In line with GDPR, your records are kept securely for 7 years following the end of treatment to comply with the legal requirements. After this time, your information will be securely deleted. For children and young people records will be kept for 7 years after they turn 18 years old before they are destroyed.
